Transition Open House

Join us at Lifelong Access on Thursday, December 4th, 2025, from 3:00 p.m. to 5:00 p.m. at our Normal, IL location (2000 Jacobssen Dr.) for our Transition Open House! This is a comprehensive event designed for students, parents, and guardians to learn about the services and programs that facilitate a successful transition from school to work and adult life.

What You Can Do at the Open House:

• Meet the Staff: This is a great opportunity to meet the dedicated staff, including Vocational Coordinator Julian Ortiz, Job Facilitator Dorothy Gordish-Mundy, and Transition Assistants Tom Schniedwind and Addie Perry.
• Learn About Future Transition Programs: Get informed and pre-register for upcoming programs designed to build skills and confidence:
  • Fall 2026 Transition Club
  • Spring "Breakers" Program
  • School to Work Summer Program
• Explore Lifelong Access Services: Learn more about our core offerings, including Supported Employment and Behavioral Health. Our goal is to ensure "Services needed. Barriers removed."
• PUNS/CISA Information: Speak directly to a representative from CISA to learn more about the Prioritization of Urgency of Need for Services (PUNS) database.
• Consent Form Signing: Parents/guardians of students participating in the Spring Transition Club are specifically encouraged to attend to sign yearly consent forms and solidify schedules for the next semester.
