At Lifelong Access, we provide essential support for a fulfilling life in the Bloomington-Normal community. We start by identifying your loved one's specific goals, personality, and needs. This information forms their Person-Centered Individual Service Plan (ISP). The plan is a detailed set of instructions we follow daily to ensure they are achieving their desired outcomes in the most supportive and independent setting possible. Our goal is to promote independence and a genuine sense of belonging within the community.
Direct and Reliable Staff Support
Our Direct Support Professionals (DSPs) are dedicated staff members who provide the daily assistance needed for safety and growth. They handle essential needs while helping your loved one achieve personal goals:
- Daily Living: Support with managing medications, personal finances, grocery shopping, and meal preparation.
- Community Integration: Staff help individuals find and connect with desired community activities, build relationships, and develop natural supports (unpaid, typical relationships).
Supported Living Options
We offer two distinct models based on the level of support an individual requires:
24-Hour CILA
This model provides comprehensive care and security around the clock.
- Constant Support: Staff are awake and present all night, every night, ensuring reliable support and safety.
- Settings: Individuals live in comfortable, shared homes or duplexes across the area, with settings ranging from 4 to 8 people.
- Privacy: Every individual has their own private bedroom.
Intermittent CILA
This option is designed for individuals who are largely independent but require scheduled, specific assistance.
- Autonomy: Residents live in their own personal apartments.
- Targeted Support: Staff visit on a schedule throughout the day to assist with needs like:
- Medication management
- Scheduling and attending medical appointments
- Budgeting and financial skills
- Community outings
- There are no overnight staff on site. This model is suitable for those needing minimal staff intervention.
